Student Incident Reporting Form
New Milford Public Schools is dedicated and maintaining a positive learning environment where all students are welcomed and feel safe in school, socially, emotionally, intellectually, and physically.  

Parents or guardians of students may fill out the form below should bullying be suspected.

This form will be shared with the building administrator. Once investigated, the administrator or their designee will follow up with the individual who submitted the report.
