NY/ACDA 2018 Middle School Honor Choir
Nomination Form - to be submitted by the Nominating Choral Director
-Choral Director must be an active member ACDA or NYSSMA
-Singers must be in 7th or 8th grade during the 2017-2018 school year
**6th Grade singers with changing voices are also eligible as Tenors and Basses.  
-Application and application fee deadline is May 1, 2018
-ACDA members: $15 non-refundable application fee per recommended singer.
-NYSSMA members (non-ACDA): $20 non-refundable application fee per recommended singer.
