London Autism Group Charity is asking members of our community to complete a short anonymous wellbeing survey.

We are doing this to better understand the wellbeing of people who use or connect with our services, and to understand whether our support is helping. The information will also help us improve what we do and report anonymised impact to funders, including Global’s Make Some Noise.

This form is anonymous. Please do not include your name, email address, phone number, address or any information that could identify you.

We will use the anonymous responses from this survey to understand community wellbeing, improve LAGC services, and report anonymised findings to funders and partners. We will not publish anything that identifies you.

Because this form is anonymous, London Autism Group Charity will not know who has completed it and will not be able to respond to you personally based on your answers. If you need support from us, please email contact@londonautismgroupcharity.org instead.
