Dear Participant, This survey is for divorced families who are currently or have been involved in music therapy. This letter is a request for you to take part in a research project to assess how music therapy affects divorced families and familial relationships in the U.S. This project is being conducted by Mikayla Teets, a student in the AP Research course in the Capstone Program, with supervision from Mr. Caskey (contact at firstname.lastname@example.org) my AP Research teacher at University High School. Your participation is greatly appreciated and will approximately take 10 to 15 minutes to fill out the survey.
Your involvement in this project will be kept as confidential as legally possible. All data will be reported in the aggregate. You must be 18 years of age or older (for adults) and 6 years of age or older (for children) to participate. I will not ask any information that should lead back to your identity as a participant, and your answers will be completely anonymous. Your participation is completely voluntary. You may skip any question that you do not wish to answer and you may discontinue at any time. Your standing will not be affected if you decide either not to participate or to withdraw.
