This application questionnaire assumes the candidate has completed the Transferring Ministers Application, received permission from a General Superintendent to transfer into the Wesleyan Church and has completed all courses required by the Education and the Ministry Department. The answers on this questionnaire will enable district leaders to judge the readiness of the candidate for validation of their ordination in the Wesleyan Church. It should be completed carefully and fully.
