For directors bringing more than one ensemble you will need to complete this form for each ensemble.

Remember that if you are attending for Comments only, there are no literature requirements.
You can change from medals to comments only or vice versa up to April 4th - if you have a change, please notify the site host when you arrive at the festival location.
