Consent Form for Online Survey
You are invited to participate in an online survey on the status of medical physics equity and gender issues.

This survey aims to collect data to identify medical physicists' profiles and equity and gender issues in the professional and educational environment. Based on the participants' feedback, we seek solutions to overcome structural barriers to gender equality in medical physics educational and professional aspects.
Analyzed data will be presented at relevant professional events/publications, preserving the participants' identity.
Your participation in this survey is entirely voluntary. You are free to decline to answer any particular question you do not wish to answer for any reason. You can withdraw anytime during the questionnaire process.
All information received in this questionnaire will be used for educational and research purposes only and treated in strict confidentiality.
The survey will last about 10-15 minutes.
We strongly encourage you to participate in this survey. Although there is no direct personal benefit, your responses may be valuable for our profession on a global scale and in various aspects of medical physics, mainly to overcome structural barriers to gender equality.

The survey organizers care about your privacy and are committed to protecting your personal information in accordance with fair information practices and applicable data privacy laws. Survey data is only visible to the administrator; all records are anonymized after collection. Please note that all your personal details will be used exclusively for control purposes and to exclude duplication.
The non-personal data will be aggregated to provide broad demographic information, such as the geographic location of participants. Collecting this type of information allows us to analyze trends and statistics and provide better service and security.
