Please fill out this form to request a Dark Skies Teacher Tool Kit. By requesting a Dark Skies Teacher Tool Kit, you are agreeing to the following terms and conditions:

- Two Dark Skies Tool Kits are available to rent each month. Rental reservations are made on a first come, first served basis.
- The rental period is one calendar month.
- Multiple classrooms/groups at a school/organization may use the kit during a single rental period as long as their usage is reported through the reservation request form.
- The kit or its contents MAY NOT leave the school/organization premises unless pre-approved by Tracy Aviary staff.
- Tracy Aviary staff will drop off the kit to you at your location on the first weekday of the month and provide a 10-15 minute orientation on proper care of the kit. Staff will pick the kit up from your location on the last weekday of the month. They will check the contents of the kit before they leave the premises.
- You will be held liable for replacement of any broken or missing items.
