Sutton Mencap is working with the Learning Disability Health Action Group to find out more about people’s experiences of Annual Health Checks.

The aim of the survey is to look at how well annual health checks are being carried out and whether there are any areas that we can recommend for improvement. You can complete this survey yourself, or get someone who knows you well to complete it with you.
