How to Register:
1.) Fill out the form below 
2.) Submit payment in one of the following ways:
         a.) Send a check for $125 made payable to "PTLIBA" to
                    PTLIBA c/o Bryan Morreale
                    329 Hedgerow Dr.
                    Venetia, PA 15367
         b.) Bring check for $125 with you (same details as above) to the first day of camp
         c.) Can pay via credit, debit, or apple pay when you check in on the first day of camp 
                    (note that a $4 processing fee is added to any electronic transactions)
