2024 Hall of Fame Nominee Information - Mixed Division

Please complete this form about your playing history to accept your Mixed division Hall of Fame nomination. This information will be used by the Peer Pool to evaluate candidates to be placed on the slate for the Hall of Fame Class of 2024.

Once you complete the Nominee Information form, email us a HEAD shot photo and an ACTION shot, if you have one (jpeg or png with photographer name, if available). Refer to the Selection Process + Timeline document on the UHoF website for competitive history search tools.

At the end of Peer Pool voting (late July 2024), you will be notified of your standing and if any additional information is needed.
