Please Read Carefully: Legal Release Of Liability In consideration of the coaches furnishing services and/or equipment to enable me to participate in free fitness activities and games, I agree as follows:I fully understand and acknowledge that: (1) Risks and dangers exist in my use of boot camp equipment and my participation in boot camp activities; (2) My participation in such activities and/or use of such equipment may result in my injury or illness, including, but not limited to bodily injury, fractures, partial and/or total paralysis, eye injury, heat stroke, heart attack, death or other aliments that could cause serious disability; (3) These risks and dangers may be caused by negligence of the coaches, participants, accidents, the forces of nature or other causes; and (4) By my participation in these activities and/or use of equipment, I herby assume all responsibility for any losses and/or damages, whether caused in part or in whole by any other person. I, on behalf of myself, my personal representatives and my heirs, hereby voluntarily agree to release, waive, discharge, hold harmless, defend and indemnify ____________, Herbalife, all participants and coaches from any and all claims, actions or losses for bodily injury, property damage, wrongful death, loss of services or otherwise which may arise out of my use of boot camp equipment, or my participation in boot camp activities. I specifically understand that I am releasing, discharging and waiving any claims or actions that I may have present or in the future for the negligent acts or other conduct by owners and coaches. I hereby grant full permission for any photos/media, which may include me, to be shared online or in the store. I HAVE READ THE ABOVE WAIVER AND RELEASE AND MY SIGNING IT AGREES THAT IT IS MY INTENTION TO EXEMPT AND RELIEVE SO FLO NUTRITION, OR ANY OTHER COACH FROM LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE, OR WRONGFUL DEATH CAUSED BY NEGLIGENCE OR ANY OTHER CAUSE. I AM PLAYING AT MY OWN RISK.
