Before filling out this form, please make sure you have all the documentation needed to start the placement process.   We are automatically notified when you submit this form.

Placement request with placement details (submitted by university - can come separately and can be in email format)
Student Application (if your university has your student complete for placement)  
Resume
Student's transcripts
Philosophy of Education (1 page only)
3 letters of reference (please submit together vesus one at a time.  Thank you.)
Certification (optional)

Please be sure the student's university email is provided (versus a personal email).

A TB test IS NO LONGER REQUIRED.

A National processed fingerprint must be provided using Parkway's code.   Please email studentteacher@parkwayschools.net for instructions.

Please note these deadlines in order for an application to be considered:
Fall placement (August - December):  APRIL 15
Spring placement (January - May):  NOVEMBER 15
