OHHS Athletic Event Travel Release
For the student-athlete to be released from riding OHLSD transportation for the athletic event this form must be submitted no later than 3pm on the day of the athletic event, or for a Saturday or Sunday event, it must be submitted by 3p on the Friday before the event.  The head coach must be notified upon the student's departure from the event to ensure open communication and safety for all involved.

Please only submit a release form on a game-by-game basis a day or two prior to the event.  DO NOT submit release forms for the entire season at once!

My student or I will notify the coach upon my student's departure from the game/meet to ensure open communication and safety for all involved.
