Acknowledgement of Risk / Informed Consent (minor)

Participants registered in the REC SOCCER Program of the Tiger Hills Recreation District must, at all times, abide by the guidelines and regulations set forth by the program. The Tiger Hills Recreation District will not be held responsible for injuries, accidents, sickness, or loss of property, which may result from participation in the program.

I have reviewed the description of the REC SOCCER Program of the Tiger Hills Recreation District and feel that I have sufficiently informed myself about the nature of the program and the activities involved.

I acknowledge that there are elements of risks and hazards associated with participation in the program, including but not limited to:

· Physical exertion and rapid movements which may cause injury to bones, joints, ligaments, muscles.

· Impact or contact with other participants, instructors, stationary objects or equipment used in connection with the program.

· Dangers arising from facility and equipment use, or participants of varying skill levels.

· Contraction of illness or contagious disease including, but not limited to, COVID-19.

· Loss of or damage to personal property.

The risk of sustaining injuries or illness results from the nature of the activity and can occur without fault of the participant(s), or the Tiger Hills Recreation District, it's employees/agents or the facility where the activity is taking place. By choosing to have your child(ren) take part in these activities, you are accepting the risk that your child(ren) may be injured or become ill and bear responsibility for any injury or illness that may occur. The chance of injury or illness occurring can be reduced by carefully following instructions at all times while engaged in the activity. Accident insurance is not provided and parents/guardians are encouraged to seek insurance coverage from a provider of their choice.

In consideration to the above, I hereby give my permission for, the registered participant(s) named above, to participate in the Tiger Hills Recreation District REC SOCCER Program; I acknowledge and accept the risk inherent in the requested activity; and I agree to hold harmless the Tiger Hills Recreation District its agents and employees, from any and all claims and actions arising as a result of their participation in the program.
