Liability Release and Waiver and Code of Conduct
Liability Release & Waiver: I hereby waive the right to make any claim against the Illumination Foundation, its board members, employees or volunteers, for injuries or damages, charges or expense, including attorney’s fees which might be sustained as a result of my participation or transportation therewith in the above activity/activities. This waiver is given in partial consideration for permission granted by the Illumination Foundation to participate in the activity or activities. I further understand that the Illumination Foundation does not provide any form of insurance for program participants.

Code of Conduct: All employees, volunteers, agents and board members may not solicit or accept gifts or gratuities for their personal benefit.
All volunteers and staff have an obligation to report any actual or suspected wrongdoings including, but not limited to:
Conflicts of Interest
Illegal Acts
Erroneous grant reporting
Erroneous facts in grant submissions

The following actions are to be taken to report a violation:
Wrongdoings should be reported to your direct supervisor.
If for any reason it is not possible to report the suspected violation to your supervisor, you should report it to the Director or the Executive Director.

Illumination Foundation strictly forbids any retaliation against anyone who, in good faith, reports suspected wrongdoings.

I hereby release Illumination Foundation from liability, claims and demands of whatever kind or nature, either in law or equity, that arise or may hereafter arise from my activity volunteering. Volunteer Handbook

I have received my copy of the Volunteer Policies & Procedures for Illumination Foundation and I understand and agree that it is my responsibility to read and familiarize myself with the policies and procedures contained in this handbook. I further agree to be bound by the terms contained in this handbook. I also understand that a breach of the terms and policies may result in the termination of my volunteer position.

I understand the contents of this handbook constitute only a summary of the volunteer policies and procedures in effect at the time of publication. I understand this handbook should not be construed as creating any kind of “employment contract” and Illumination Foundation has the ability to add, change or discontinue policies or other working conditions, as it deems appropriate without obtaining another person’s consent or agreement.

I understand that in the course of my voluntary service for IF, I may learn certain facts about residents, staff, and clients of IF, which are of a highly personal and confidential nature. I understand that all such information, including the identity of the individual, must be treated with total confidentiality and must remain confidential even after my service with IF terminates.

I understand that my supervisor, the President/CEO or any other executive member of management are willing to explain any portion of the handbook that I may wish to discuss or about which I have a question.

I am offering my volunteer services freely without pressure or coercion and without promise, expectation or receipt of compensation for services rendered.
