Please fill out this form for any absences in advance. This includes any rehearsals, performances, or required events, regardless of the time of year. Filling out the form does not guarantee an "excused" absence, it is to document the reason for the absence. Please refer to the band handbook for more information on our attendance policy. Following the absence request, a director will follow up with the student and/or parent.
