TDS Extended Care Survey
In order to plan for students and staffing for the Extended Childcare Program ( ECCP), we ask that you complete this short questionnaire. This information is being gathered for parents who plan to use the ECCP on a consistent basis at least two days a week.

We will use the information to determine parent needs as it compares to our staffing as well as our capacity to adhere to health and safety guidelines. Drop in attendance will need to be approved by the site director.

The information gathered from this link is not in lieu of the enrollment paperwork. That paperwork will be shared the first week of school.

As long as school is open in the traditional school model, our intention is provide childcare service. If it were to become necessary for the school system to implement the hybrid or virtual scenario, childcare services will be interrupted and possibly canceled during those scenarios.

This survey is only for grades K-5. If you have a PreK child who needs childcare, please let the PreK teacher know.

You will be contacted by site director the week of July 27 - 31, 2020 in regard to your child's attendance in the ECCP. Our goal is to be provide childcare for all families who need our services.
