Grievances & Complaints
This form is for parents or caregivers of clients to submit a formal complaint or grievance to Radiant Kids Therapy regarding their child's services or anything related to Radiant Kids Therapy's provision of those services. 

In most cases, when appropriate, we ask that parents/caregivers first attempt to address the issue directly with the team member involved.  When this does not lead to a resolution, you may reach out to us directly at radiantadmin@radiantkidstherapy.com or fill out this form.

A representative from our Management Team will respond to you to confirm receipt of this within 72 business hours.  If a resolution is applicable or needed, we will investigate the issue(s) described above and will reach back out to you within 30 days of the submission of this form to follow up and discuss further, as needed.
