22-23 WP Change in After School Plans
Please use this form to submit any changes to your student's after school plans. Changes can be submitted up to 1:00 pm on the day of the change in dismissal.  

1.  Please do not submit changes after 1pm on the day of the change; after 1 pm you should contact our front office at 404-370-4490.  

2. A form must be filled out for each sibling/student.
3. The school does not notify after school providers of a student's absence.
