Transcript Request Form
Please complete this form to request your NorthStar Academy transcript. Please note that official transcripts will not be sent if the student has a current due balance, and final transcripts will not be sent until the student's balance is paid in full.

The processing time for transcripts is as follows: Once your transcript request has been submitted, you will receive an email within 1 business day regarding the payment that is due before your transcript can be sent. Once you make your payment, please respond to that email with confirmation that payment has been made so that there is no delay in getting the transcript in the mail. Once you have confirmed payment, please allow at least 48 hours for processing.
