Please complete the form below to register your child for kindergarten for the 2022-2023 school year.  

Your child must be five years of age by August 1st, 2022, in order to register.  The following information will be required to complete registration when your child comes for kindergarten screening.  

1) Official birth certificate
2) Up-to-date shot record
3) Current kindergarten physical
4) Social Security card
5) Medicaid card, if applicable
6) Completed registration paperwork that will be given to you at screening.

Screenings will be held May 17th-19th. Parents/Guardians will be contacted by a staff member to make a screening appointment. If you have any questions, please call Westside Elementary at 479-497-1088.
