I. INTRODUCTION:

This pre-acceptance informed consent document is designed to ensure transparency, fairness, and clarity regarding the responsibilities, expectations, fees, charges, and policies of the Kentucky Phoenix Project's Recovery Housing Program (RHP).

II. ACKNOWLEDGEMENT OF RHP MANUAL:

I acknowledge that I have received and reviewed the RHP Manual. I understand and agree to abide by the rules, policies, and procedures outlined within the manual and all other policies of the Kentucky Phoenix Project Recovery Housing Program.

III. MEDICATION-ASSISTED TREATMENT (MAT) REQUIREMENTS:

As a condition of my residency within the Recovery Housing Program, I am aware that there is a policy requiring a shift from oral medication-assisted treatment (MAT) to an injectable, long-acting form of MAT. I understand that any exceptions to this policy must be supported by a written statement from a licensed medical provider, which the Kentucky Phoenix Project will consider. I agree to adhere to this requirement as an integral part of my commitment to recovery.

IV. PARTICIPATION IN SERVICES BASED ON ASAM LEVEL OF CARE ASSESSMENT:

I understand that as a resident, my participation in services, at a provider of my own choosing, will be based on an ASAM (American Society of Addiction Medicine) level of care assessment. This may include, but is not limited to, services tailored to my specific recovery needs. Proof of participation in these services must be provided to the Kentucky Phoenix Project on a weekly basis.

Additionally, I must:

Engage in employment, volunteer work, or education as a demonstration of progressing towards societal reintegration and personal responsibility.
Demonstrate continuous progress toward my recovery goals as outlined in my personal recovery plan.

V. RESIDENT FEE AGREEMENT:

I agree to pay a weekly program fee of $100. Participation in certain recovery-oriented activities will result in a reduction in the weekly fee as specified in our program details. A reduction of $75.00 in weekly fee's occurs for a resident Intensive Outpatient Services (IOP) and reduction of $50 is made for residents receiving regular outpatient services (OP).

VI. SERVICE PROVIDER CHOICE AND ATTENDANCE:

I acknowledge that I am required to attend services as part of my recovery program. However, I am free to choose any service provider that meets the requirements of the RHP. My choice of service provider will not affect my residency status as long as I am compliant with the attendance and participation requirements.

VII. RESIDENT FEE REFUND POLICY:

I have been provided and understand the "Kentucky Phoenix Project Recovery Housing Program - Resident Fee Refund Policy." Refund policy is viewable in the RHP Manual linked above. I acknowledge that fees, once paid, are generally non-refundable, with few exceptions as outlined in the policy.

VIII. COMMUNAL RECOVERY HOME ENVIRONMENT:

I understand the risks and benefits of staying in a communal recovery home environment. This includes potential conflicts with other residents, the benefits of mutual support, shared responsibilities, and potential triggers that may arise from living in a communal setting.

IX. PRIVACY AND CONFIDENTIALITY:

I acknowledge that my personal information and records will be kept confidential, and I understand the circumstances under which this information might be shared or disclosed in compliance with state and federal regulations.

X. DRUG AND ALCOHOL TESTING:

I understand that as part of the Recovery Housing Program, I may be subjected to random drug and alcohol testing to ensure a safe and drug-free environment for all residents. Associated fee's are explained in the RHP Orientation Manual, but in general testing is free to the resident.

XI. TERMINATION OF RESIDENCY, EVICTION PROCESS, AND DEPARTURE FROM PREMISES:

I understand that any violation of the policies, rules, or procedures of the Recovery Housing Program, including but not limited to non-compliance with MAT requirements, non-participation in required services, non-payment of fees, relapse, or aggressive behavior, may result in the immediate termination of my residency. By agreeing to the terms of this program, I acknowledge that my residency status is contingent upon adherence to these specific program requirements, which may not afford the full procedural rights typically associated with residential eviction processes under local, state, or federal housing laws. This acknowledgment does not waive my rights to due process under the law but clarifies that certain aspects of the program's policies are designed to maintain the safety and integrity of the recovery environment, which may expedite the process for addressing non-compliance or behaviors that compromise the program's objectives.

Furthermore, in the event of termination of my residency for any reason, I agree to vacate the premises immediately, or within a timeframe deemed reasonable by the Kentucky Phoenix Project, to allow for the continued safety and well-being of all residents and staff. This agreement ensures a swift and orderly transition that respects the recovery process and community living environment of all program participants.

XII. PEER SUPPORT SPECIALIST AVAILABILITY:

I acknowledge that peer support specialists are available for support within the house Monday through Friday, from 8 AM to 4 PM. Their presence is to provide additional recovery support and guidance during these hours.
