RELEASE, ASSUMPTION OF RISK AND  INDEMNIFICATION AGREEMENT (Minor) 

CAUTION—This is a release of legal rights. Read and understand before signing. 

I and the adult parent or guardian of [named below], a minor child under the age of eighteen years, and I consent to his/her participation in tennis and fitness activities in and upon real property and improvements known as Bloomington Tennis & Turf, 4101 Wicker Drive, Bloomington, McLean County, Illinois (“the Property”). As a condition of him/her participating in tennis and fitness activities on the property and in consideration of allowing his/her participation in them, I hereby agree as follows: 1. I, individually, and on behalf of [named below] , and on behalf of his/her heirs, successors, assigns, and personal representatives, hereby release, acquit and forever discharge Bloomington Tennis & Turf, and its principals, heirs, assigns, successors, employees, agents, and representatives (in their official and individual capacities), from any and all liability whatsoever for any and all damages, losses or injuries (including death) he/she sustains to his/her person or property, or both, including but not limited to any claims, demands, actions, causes of action, judgments, damages, expenses and costs, including attorneys’ fees, which arise out of, result from or occur during, or are connected in any manner whatsoever with, his/her participation in tennis and fitness activities in or on the Property, wherever, whenever, or however the same may occur. 2. I, individually, and on behalf of [named below] , and on behalf of his/her heirs, successors, assigns, and personal representatives, hereby acknowledge that, by participating in tennis and fitness activities in or on the Property, he/she is engaging in a potentially dangerous activity and freely and voluntarily assume all risk to his/her person resulting from his/her participation. 3. I understand that this agreement is intended to be as broad and inclusive as permitted by the laws of the State of Illinois and I agree that if any portion is held invalid, the remainder of this agreement will continue in full legal force and effect. I further agree that the venue for any legal proceedings arising from or connected to this agreement shall be in McLean County, Illinois. 

I hereby grant Bloomington Tennis and Turf permission to use my/my child image, likeness, or voice in photographs, videos, or other digital media (“media”) on its social media platforms (e.g., Facebook, Instagram, TikTok, Twitter) and website for promotional purposes. I waive any right to compensation, acknowledgment, or approval of the media’s use. I understand that Bloomington Tennis and Turf may edit or alter the media but will not do so in a malicious or explicit manner.

By signing below, I release Bloomington Tennis and Turf from liability related to the use of my image.
