Please fill out this form for each lost tag with required information in order to document the lost tag and new replacement tag and/or to request a new CBJLS tag (if applicable).
Swine:
You must fill out the below form if you lose your CBJLS Tag or the 840 Premise ID Tag. If you have lost the 840 Premise ID tag you will complete this form after you require/acquire the new 840 Premise ID Tag from your breeder. CBJLS will not be supplying you with a new tag.
The new 840 Premise ID number reported must match tag in the hog's ear before going over the scale. If you lose your CBJLS tag we will re-tag it on Sept 8th when going over the scale but it must match the 840 Premise ID tag we have on file. Only animals with lost tag forms submitted will be allowed to get a new tag and go over the scale.
Sheep:
You must fill out the below form if you lose your CBJLS Tag or your Scrapie ID Tag. If you lose your Scrapie ID tag you must get the new one from the breeder and report the number on this form (only submit form once you have new Scrapie ID tag). If you lose your CBJLS tag we will re-tag it on Sept 8th when going over the scale but it must match the Scrapie ID tag we have on file. Only animals with lost tag forms submitted will be allowed to get a new tag and go over the scale.
Beef:
You must fill out the below form if you lose your CBJLS Tag. Your CBJLS tag we will re-tagged on Sept 8th when going over the scale. Only animals with lost tag forms submitted will be allowed to get a new tag and go over the scale.
