Mentor Application Form
Thank you for expressing interest in being a Mentor on the Trailblazer Mentorship Platform by Talent Stacker.
Please read the following information carefully.
Form Submission:
By completing this form you are agreeing to dedicate a single 1-hour session per week with the Mentee(s) you select for 4 consecutive weeks. You also agree to be punctual, communicative, and respectful in all interactions. All mentorship on this platform is provided on a volunteer basis.
Submitting this form does not guarantee you will be selected as a Mentor.
If you continue the mentorship relationship beyond the delineated timeframe that is between you and your Mentee(s).
Mentor Resources Needed:
On the mentorship platform, you will have a field to put in your calendar scheduling link. If you do not already have a calendar scheduler - we recommend Calendly or SimplyMeet.
Next Steps:
After submitting this form you will receive an email confirming submission with additional information about how to be prepared for the most effective mentoring experience. We will keep you informed on upcoming Mentorship Cohorts so you can be ready for your Mentee Requests to come in. If you are selected as a Mentor you will be sent Mentor Profile creation instructions so that you can get set up prior to the Mentees joining and seeing you as Mentor available on the platform. Once a Mentee requests you as a Mentor you will receive a Booking Request which you can choose to accept or decline. Typically you would decline if you are at capacity or simply don't feel you are a good fit for the expressed needs of the Mentee. When you Accept a Mentee they will be sent your calendar scheduling information and your Mentorship Experience begins!
