If your child is absent from school, please fill out this form to notify the front office. Please fill this out for each child who is absent. Without an absence note, your child's absence will be marked as "unexcused". 

如若您的孩子无法上课要缺席，请填报这份表格，以便通知学校的前台办公室。请为每一位缺席需要请假的孩子填报一份。在没有请假条的情况下，您的孩子缺席会被记录为“无故缺席”.

Si su hijo está ausente de la escuela, complete este formulario para notificar a la oficina principal. Por favor complete esto para cada niño que esté ausente. Sin una nota de ausencia, la ausencia de su hijo se marcará como "injustificada".
