Pets require a minimum of one physical exam each year, as well as, vaccinations and blood work per your veterinarian’s recommendation. A pet may also need additional veterinary visits and care should he or she become ill. We estimate the average annual medical cost of a cat to be $500-$800 for a young, healthy pet, more for a senior and $800 and up for a healthy dog, and more for a senior. Our Policy Regarding the Age of Potential Adopters Adopting a pet is a lifetime commitment, so that the pet receives attention, care and respect for the rest of his life. We need to feel comfortable that each potential adopter can provide a stable environment for the pet, has the ability to pay for medical care, as well as basic necessities, will be in good enough health to care for the pet. For these reasons, we apply additional scrutiny to potential adopters who are less than 21 (twenty-one) years of age or older than 70 (seventy) years of age and require that these individuals provide a “back up” caretaker or adopter for their newly adopted pet. We treat each case individually and are happy to work with you to find a solution. Potential Adopters Less than the Age of 21, A potential adopter who is at least 18 (eighteen) years of age, but less than 21 years of age, is less likely than an older person to have the financial means to care for a pet, to have a stable living situation to ensure that the pet will have an appropriate home for an extended period of time. For these reasons, our Adoption Counselors may ask questions that seem intrusive. Potential Adopters Older than the Age of 70 is less likely than a younger person to outlive a young pet, to remain healthy enough to provide a lifetime of care for a pet. For these reasons, our Adoption Counselors will ask you to consider older pets and may ask intrusive questions. Please list the name of the co-adopter and their information on your adoption contract to allow transfer of ownership if necessary.

I understand that Pura Vida reserves the right to deny an adoption request for any, or no, reason and that Pura Vida may choose not to reveal specific reasons. I affirm that all of the information on this application is true and complete and that any incorrect information or omissions are grounds for denial. I hereby grant permission for any veterinary offices listed above to speak to an employee/volunteer of Pura Vida to discuss information related to my ownership and care of pets. I have read and understand the policy regarding the Age of Potential Adopters. Pura Vida will confirm that this person is fully aware of their commitment to adopt and care for your pet in the event that you are no longer able to do so for any of the above reasons. In the event I can no longer provide necessary care and need to surrender my pet I understand I must contact the organization (Pura Vida) directly regarding the return of my pet.
