CONSENT:
I hereby declare that I am 18 years old and above and my involvement in this program through answering the different questions in this registration form. I have read and agree to the conditions stated as well as the following:
(1) I am informed of the purpose of the workshop;
(2) I participate willingly;
(3) I have the freedom to remove myself from the workshop; and
(4) I would like to take part in this workshop.
Personal information acquired through this Google form is entered and stored in the approved information and communications system, with only authorized persons having access. Moreover, the data generated and kept in the site will only be utilized solely for this project and its data reporting. Without the participant's approval, the implementing team will not release any personal information.
Idinedeklara ko na ako ay 18 taong gulang o higit pa at ako ay magiging bahagi ng programang ito sapamamagitan ng pagsagot sa form na ito. Ako rin ay may sapat na kaalaman sa programang ito at sumasangayon base sa mga impormasyong nakasaad sa simula ng form. Bukod dito ako ay:
(1) Nabigyang kaalaman sa layunin ng workshop;
(2) Boluntaryong nakikibahagi;
(3) May kalayaang tumanggi na makibahagi sa workshop; at
(4) Gusto kong maging parte ng workshop na ito.
Ang mga personal na impormasyong itinala sa form na ito ay nakalagay sa aprubadong sistema ng opisina at tanging mga taong awtorisado lamang ang pwedeng mag-access at gumamit nito. Bukod dito, ang mga nasabing impormasyon ay gagamitin lamang para sa implementasyon at reporting sa proyektong ito. Hindi kailanman isasapubliko ang mga nakatalang impormasyong kung walang pahintulot ng indibidwal na nagmamay-ari nito.
