Application form for participating in the contest of young reporters "Welcome!" 2022
Questionnaire can be filled by the participant on his own or with the help of a senior.  For each work a separate application should be filled.

For the category "Photo" you need to write here some brief information about people, events, objects represented on the photo, and also the date of shooting and location. Similarly, for the nomination of "Video" (if that is not explained in the video).
The teacher and/or organization that prepared the participant (the head of the media team, a school teacher, etc.) - if applicable. With the indication of the educational institution and e-mail address.
