This form is to allow outside visitors the opportunity to request entry into the UTM Fine Arts Building. Non-essential requests will not be approved. All requests will be vetted on a case-by-case basis by the department chair. Please see the latest safety policies for the UTM campus on the Covid-19 website: www.utm.edu/corona. Requests for private lessons inside Fine Arts cannot be approved at this time.

Due to Covid-19, this form must be filled out within two weeks prior to the intended visit. If the visit is delayed for some reason, a new form will need to be submitted and approved.
