We the undersigned, legal parents or guardians of minor verify that the minor, player listed above is in good health and able to participate in this event. We recognize that all sports have some risk of injury and hereby release all organizers, volunteers, coaches, Kokomo School Cooperation staff/officers, and sponsors from any and all liability claims for injury, illness and loss sustained by the participant while playing, practicing and travelling to and from this event. We (participants and guardians) listed above in the form assume all risks and absolve, indemnify and hold harmless of any and all liability or damage, injury or expense of any kind arising out of or connected with this camp. I also confirm that the information on this form is true and correct and that no participant under my custody is eligible to participate without my written consent and signature as the legal parent / guardian. Photo Release I hereby give permission for my child to be photographed during the InspireHER Camp. I understand that although my child’s photograph may be used for advertising and on social media, her identity will not be disclosed. I do not expect compensation and that all photos are the property of the organizers of this camp. *
