Discontinuing a Jive Dance Class
We are so sorry to see you go! We hope to see you again soon. Please fill out the following form for each dancer in your family that will be discontinuing their class at a Jive dance studio.

What day and time (or Company) is the regularly scheduled class that this dancer will be discontinuing?

We understand that there are many reasons to discontinue a dance class. Please help us to know why this dancer will be discontinuing this class so we can always be improving our dance studio experience. Please pick the best reason below that explains this decision to discontinue this dancer's dance class.
Dancer is moving out of the area
Dance tuition is not fitting in with our current budget
