• Medical & Allergy Information – Helps us provide a safe and supportive environment.
• Consent & Agreement – Confirms your understanding of our policies and procedures.
• Pool Information – Assists us in preparing for your child’s lessons.

This form should be read in conjunction with our Terms of Service & Liability Waiver. Please ensure you review all information carefully before proceeding.
