Play it Forward provides youth ultimate players in need of financial assistance with a free USA Ultimate membership.

This form should take about 5 minutes to complete.

Players are eligible if they have not yet graduated from high school and are planning to participate in a USA Ultimate sanctioned or championship event.

If you are applying for more than 1 but fewer than 5 players, please fill out separate applications for each player.
