The data submitted via this form will be kept private and confidential, and the details required are used as follows:
1. E-mail address for forwarding of confirmation message; 2. Student number, name, and college for cross-checking with the CRS if the applicant is undergoing disciplinary proceedings or corrective measures;3. Gender for ensuring gender equality in terms of the form's accessibility;4. Purpose of clearance for advising applicants on additional details and/or requirements that may be needed on or before the schedule of release of applicant's clearance, and;5. Name and student number for printing out the student identification details in the clearance.
INSTRUCTIONS AND GUIDELINES
1) Fill in all the fields in the form2) After submitting, a confirmation e-mail will be sent to the address you gave below. The confirmation e-mail will contain the details you typed into the form, and the timestamp showing the date and time your application was received. It will also contain instructions on claiming the clearance. If you have not received any confirmation message in your address, kindly send an e-mail to email@example.com with the details of your application (approximate date and time of application, name, student number, college, etc.) to inquire if your application was received by the system.3) The cut off time for applications received is 12:00 noon. Clearance applications received prior to the cut-off time (i.e. on or before 12:00nn of that day) will be processed the same day. The corresponding clearances can be claimed starting 2:30pm.4) Any application received (as per the timestamp) after the cut off time will be processed the next working day, the corresponding clearance of which can thusly be claimed starting 2:30pm of that day. For example, if your application was received Monday at 12:30pm, then your clearance application will be processed on Tuesday (assuming it is a regular workday), and the clearance can then be claimed starting 2:30pm.5) Applications filed during non-working days (weekends, holidays, declared nonworking days) will be processed the next working day, and the corresponding clearance can be claimed starting 2:30pm of that day. For example, if your application was received Saturday at 8:00am, then your clearance application will be processed on Monday (assuming it is a regular workday), and the clearance can then be claimed starting 2:30pm.6) Clearances not claimed within the day it was processed can be claimed on Mondays to Fridays (except in cases of holidays, suspensions, and declared nonworking days), from 9:00am to 4:30pm.
