I understand that my child will be participating in activities that may include, but are not limited to, arts and crafts, slime-making, painting, games, and hands-on creative projects. I acknowledge these activities come with some inherent risks such as allergic reactions, minor injuries, slips, spills, or mess. By completing this form, I voluntarily assume all risks and release artPOP Studio, its employees, owners, and volunteers from any liability or claims arising from participation.

I authorize staff to seek emergency medical care for my child if needed and agree to take full responsibility for any associated costs.
