Census Data Information
In compliance with NDE 02-023, Superior Public Schools conducts an annual school census report.  To best collect the data, we are trying to get the most accurate information of people living within the district.  All patrons are asked to complete this questionnaire.  Please complete one form PER HOUSEHOLD that have children aged birth to 21 years of age. 

STATE FUNDING IS BASED OFF OF THIS INFORMATION, AND IT IS IMPORTANT THAT WE OBTAIN THE MOST ACCURATE INFORMATION AS POSSIBLE.

PLEASE COMPLETE AND RETURN THIS FORM TO THE HIGH SCHOOL OFFICE OR EMAIL TO sbiltoft@superiorwildcats.org.
