Please fill out the following form on the morning your child is absent so we are aware they will not be in school. According to TPS policy, upon their return to school, the student must submit an absence note dated and signed by the parent or guardian. After ten absences, excused or otherwise, a signed doctor's note must be provided for all absences, regardless of reason.
