LUP 2019 Altar-maker Application
APPLICATION DEADLINE:  FRIDAY, OCTOBER 25, 2019; 5:00PM

LUP 2019 will take place on Sunday, October 27 at The Foundation for Hispanic Education at 14271 Story Road, San Jose, CA 95127.  Community organizations are invited to apply for a space to create a unique ofrenda for Dia de Los Muertos.  Altar Set-up will take begin on Thursday, October 24 and must be completed by Sunday, October 27 at noon.

As of now, each organization will bring their own tables and necessary boxes, crates to create the levels.  However, we are working on having tables available.  This year we are in a hallway space, so ofrendas must be kept as near close to the wall as possible.  Also, since we are just new to this venue, the spaces which we are being offered are limited in number.  Assignments will be on a first-come, first-served basis.  Altar strike will be Sunday, November 3, 2019.


Altar Guidelines:
• Only painters tape is permitted
• Please do not NAIL anything onto the walls
• Confetti/glitter or similar material are NOT allowed.
• Only use battery powered tea lights; unattended flames are NOT permitted.
• .Any items that may cause an odor will need to be monitored and refreshed regularly.
• Food items must be individually wrapped in clear plastic wrap.
• Cempasuchitl flowers must be requested in advance; our order will go in on Sunday, 10/20.

Once your application has been approved, your W-9 and an invoice will be submitted for the reimbursal check of $200.  Your W-9 information should match your application mailing address and name of person that will receive the check.

I ACKNOWLEDGE, agree, and represent that I understand the nature of LUP 2019 Activities and that I am qualified, in good health, and in proper physical condition to participate in such Activities. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the Activity.
