Please use this form to sign up your child for our KIDS Summer VBS. This will be a great time for fun and community! Note that this community-focused event is free for all participants. This year, we will be playing soccer and learning about Jesus. **Please submit a separate form for each child.**

Media Release Segment:

I grant permission to FaithBridge Church to use this child's likeness, voice, and/or any other personal attributes captured in any form of media, including but not limited to photographs, videos, audio recordings, and written content, for the following purposes:

- Publication on FaithBridge Church's website.

- Inclusion in printed materials such as brochures, pamphlets, and newsletters.

- Display in presentations, exhibitions, and other promotional materials related to FaithBridge Church's activities.

- Sharing on social media platforms owned or managed by FaithBridge Church.

*If this child's image/voice cannot be used by this organization for the purpose of media, you WILL NEED to contact a staff member and we will try to accommodate you.

Liability Waiver:

I understand that participation in the activities involves inherent risks, including risk of physical injury, and I assume all related risks and voluntarily agree to this child's participation in the MegaSports Camp VBS from June 29th-July 1st.

I release FaithBridge Church against any and all claims, actions, lawsuits, damages and judgments, including attorney’s fees, arising out of or relating to my participation in the MegaSports Camp VBS from June 29th-July 1st.

I agree to comply with all safety guidelines, instructions, and rules provided by FaithBridge Church and its representatives before and during the VBS.

I have carefully read and fully understand all the provisions of this Release and am freely, knowingly and voluntarily entering into this Release.
