Account Creation Request for Sidekick to Hero
Hello Parent!

I'm excited to have your student(s) in Sidekick to Hero. It's a one of a kind program that will help them learn skills to build confidence, manage emotions, develop social skills, and achieve their goals.

To use state funding funds to pay for your membership, you have two options:
  1. Pay for it out of pocket and get reimbursed (either $25 a month or $250 a year) if your program allows you to do so (you will need to verify with them)
  2. Fill out this form to get an invoice sent to you to pay through Class Wallet ($250 a year only) OR to get an account created if you've already sent payment through Odyssey (Utah)

If you have multiple teens you would like to sign up, please fill out this form separately for each one.

If your teen does not have an email, 
  • use another parent email
  • create a free email for them on gmail or yahoo, etc.
  • (for Gmail users only) you can use the "subaddressing" feature Gmail has add a "+1" to the parent's gmail address for the teen's email. Example: parent email is "awesomeparent@gmail.com" and teen email can be "awesomeparent+1@gmail.com"
