PLEASE NOTE:

Arriving Late

Report late arrivals using this form or by calling the school attendance line at 847-398-4280, option 1, no later than 9:05am the date of the late arrival. The adult dropping the student to school must come into the office to sign the student in.

Leaving Early

Please email the teacher, mporwit@sd25.org and kkemperas@sd25.org to request an early pickup for your child before 9:05am Please include the date and time of pick up. The guardian or authorized pick up person must come into the office to sign the student out.

LEGAL NOTICE: This form is to be filled out and submitted only by the student's parent or legal guardian. In the event that a person other than the student's parent or legal guardian fills out and submits this form, AHSD25 reserves the right to take any legal action for fraudulent submission and/or take disciplinary action against any AHSD25 student(s) involved. We reserve the right to call home and verify the absence as needed.
