This needs to be filled out by the PARENT/GUARDIAN of the student athlete before the student may participate in a sport for Tenor/Veritas.
Parent/guardian forms must be completed using a parent/guardian email address. The parent/guardian forms CANNOT be filled out using the student's @mysohi.org email address.
If there is more than 1 student athlete, the parent/guardian must fill out the parent forms for each student athlete.
The student/athlete will also have to fill out the student online forms before they can participate. The student forms are different than the parent forms. The link to the student forms will be provided at the end of this form.
