School Uniform Order Form

This form is the official order form for the Ulsan Hyundai International School uniform.

Before completing this form, please visit the Admin Office to try on the sample shirts and determine the correct size for your child.

Payment must be made in cash to the Admin Office at the time of submission of this form. Ordered uniforms will be sent home with the student in 1–2 weeks.

All students are encouraged to wear their UHIS uniform shirt Monday through Friday, but it is not mandatory.
