The mission of Atlanta's student hotspotting program is for interdisciplinary teams of students to work with Grady's high-utilizers in addressing social and environmental factors contributing to frequent readmissions. By doing so, we intend to reduce barriers to management, promote engagement in primary care/preventative services, enhance self-efficacy, and ultimately-- decrease the economic burden on healthcare systems. With this intervention, patients steer the ship, and students offer support in managing health and navigating the system. We do not provide direct medical care. How does a program of this philosophy align with your personal and professional goals? (300 words max, shorter is better) *
Your answer
Please tell us about one personal or professional experience that prepares you to work on addressing systemic barriers with patients who have medically and socially complex cases. (300 words max, shorter is better) *
Your answer
We firmly believe in the power of shared work, with wonderful patient outcomes and learning opportunities coming from engaging in interprofessional exchange. Tell us (a) how you would contribute to an interdisciplinary team, (b) about a time you struggled on a team, and (c) what you learned from that experience. (300 words max, shorter is better) *
Your answer
Every student has a full plate. Student hotspotting expects a commitment of 5-10 hours/week of work with patients, communication with team members, and operational tasks (poster preparation, grant requests, engagement with the Camden Coalition's national group of hotspotters, etc.). The schedule may involve activities outside of normal business hours. Tell us about the scheduling demands you expect over the 2018-2019 school year, and how you anticipate the addition of student hotspotting to affect your work-life balance.
