Adopt - A - School Application
Thank you to a grant from the Findlay Rotary Foundation, CMF is able to offer a free field trip to qualifying Hancock County preschool, grade school, and Head Start classrooms for the 2019-2020 school year. Each qualifying classroom is allowed one free visit per year. The Activity Center is also available for free, but must be reserved when scheduling your field trip with Erica. Schools/Organizations are responsible for transportation to and from the museum.

Teachers or administrators will need to complete this form. Criteria and information is also listed on our website at www.cmfindlay.com under the Teachers tab. Once this form is completed, you will be contacted via email to inform you if your trip was approved or denied. Once you know if you are approved, you will need to schedule your visit at least a month prior to your proposed date. You can do that by contacting Erica at erica@cmfindlay.com or 567-250-9616
