Please complete the following referral/application form to express interest in enrollment to a Specialized Option opportunity (Pathfinder School of Innovation (9-12), Georgia Chaffee TAPP, Liberty High School (7-12), or The Phoenix School of Discovery) for the 2025-2026 school year. Applications DO NOT carry over from year to year.

Prior to completing the form, please review the “Specialized Options Overviews” section of this referral for details on each school. Some schools require you to visit their website to complete the application and this information is linked into this form. Once this referral has been submitted, JCPS personnel will contact you regarding placement in a Specialized Option.

*Please note that all recommendations will be based upon available enrollments per grade at each school.
