Food and Beverage Acknowledgement - No outside food or beverage will be allowed, unless approved by the East Providence School District Director of Operations. The Culinary Arts program and/or district food service company may be available if appropriate. *
East Providence Police and Fire Coverage - It is the responsibility of the applicant to contact the East Providence Police and Fire Departments to have them determine whether coverage details are necessary. On an event by event basis, the Fire Chief will determine the need for the presence of firefighters according to Title 23, Health and Safety Chapter 23-28.6 and 23-28.6-5. The applicant will be required to pay for uniformed firefighters as stipulated by the East Providence Fire Dept. The central office will contact police and fire officials directly to verify that coverage was requested and needed. *

Additional charges may result after the use of the facilities e.g., custodial, damages, etc. *
Any damages sustained to the facility and/or equipment during its use will be considered the responsibility of the organization and will be billed for any repairs needed to restore the facility/equipment to its original condition and/or for replacement. *
Total fees, unless otherwise approved, and excluding additional charges subsequent to the event, are required two (2) weeks prior to the event *

I understand that as a representative of the Applicant, I/we accept responsibility for compliance with the East Providence School Committee policies and regulations contained herein, as well as the applicable laws, payments associated wit said use. Failure to do so may result in forfeiture of application now and for future usage. *
I understand that the requested application may require changes and/or cancellation in order to accommodate school related functions. *
I understand there is a one-time, non-refundable application fee of $200.00 due at the time of application submission. *
