By completing this form, I acknowledge that I am the parent or legal guardian of the student named in this registration and that I grant permission for my student to audition for the SkoVu TV TikTok Dance Competition, presented in collaboration with Success+Me Enrichment Program.

I understand and agree to the following:

Audition Submission & Selection

Allowing my student to submit an audition video does not guarantee that they will be selected to compete in the live TikTok Dance Competition or be eligible to compete for the $750 grand prize.

Academic & Behavioral Eligibility

I understand that my student may be deemed ineligible to participate if a school administrator from their high school provides written notice stating that my student is not in good standing academically, behaviorally, or in attendance at the time of selection or prior to the event.

Audition Video Requirements

I understand that all audition videos must be:

Submitted by January 15, 2026

Shared via an unlisted YouTube link

Under two (2) minutes in length

Filmed in landscape (horizontal) orientation

Cannot contain any sexually explicit or vulgar content

Videos that do not meet these requirements may not be considered.

Public Voting & Visibility

I understand that all eligible audition videos will be posted to www.skovutv.com for public viewing and voting.

Public votes will help determine the 20 finalists selected to compete live at the event.

Finalist Registration Fee

If my student is selected as one of the 20 finalists, I understand that a $25 registration fee is required for them to compete in the live competition on March 7, 2026.

I understand that one parent or guardian is responsible for submitting the registration fee no later than January 31, 2026.

Failure to submit the registration fee by the deadline may result in forfeiture of my student’s spot.

Solo or Group Participation

I understand that students may compete as a solo dancer or as part of a group.

If competing as part of a group, I understand that registration payments and communication must still be coordinated by a parent or guardian.

Photography, Video, and Media Release

I grant permission to SkoVu TV and Success+Me Enrichment Program to photograph, record, and use video footage, images, and audio of my student in connection with auditions, the event, and related activities.

I understand that this content may be used at their discretion for promotional, educational, marketing, and archival purposes, including on websites, social media, and other media platforms, without additional compensation.

I confirm that I have read and understand the information above and grant permission for my student to participate in the audition process under these terms.
